Physical Exams
The Baumholder Health Clinic Physical Exams Section offers physical exam services for service members on a walk-in basis.
Physical Exams are now conducted 100% electronically on MODS:
- Navigate to https://www.mods.army.mil
- Once on mods.army.mil click “MHA”
- Now Click “SHPE” then locate Form 2807-1 AND 2808 and click “Start New Survey”
- Complete Both Forms electronically
- After all forms are complete, visit Front Desk of the Baumholder Primary Care, provide proof of completion of steps 1-4.
- A nurse/medic will submit lab orders, proceed to lab and if required, Radiology for chest x-ray.
- Now you will need to visit Dental, Hearing, Vision, you will have exams completed and they will provide an electronic signature inside the documents.
- You will receive a message on MHS Genesis when labs return.
- Once Lab’s return, visit Front Desk of the Baumholder Primary Care on Thursdays during walk in hours (8-11:30 a.m. / 1-4 p.m.)
- At this time, lab results will be recorded, and any questions/ concerns will be addressed.
- The Front desk staff will now schedule your Phase 2 Appointment with your PCM.
- During the Phase 2 Appointment, you will have your physical exam with your PCM, Please Wear PT uniform.
- After your Phase 2 appointment, you may then provide the completed packet and forms located on www.mods.army.mil to the Receiving party.
***Do not schedule phase two through the Appointments Line, please schedule in person through Primary Care according to step 11***
For the Phase 2 appointment, you must be 15 Minutes early, be in PT Uniform and report to the Front Desk at BLDG # 8740, Second Floor Waiting Area for checking in.
Service Members initiating Command Directed Separation Examinations IAW AR 40-501 and AR 635-200 (Chapters 5-13, 5-14, 5-17, 13, 14-12a, 14-12b and 14-12c require an Escort (SGT/E-5 or above) while being in this MTF.
