Physical Exams

The Baumholder Health Clinic Physical Exams Section offers physicals for service members on a walk-in basis. Physicals are conducted in two parts. The Soldier will report to the Physicals Section/Radiology to receive packets and further instructions. Once complete, the Soldier will return to the Physicals Exam Section to schedule part two, which will be in Primary Care.

***Do not schedule part two through the Appointments Line or at Primary Care. You must schedule in person through the Physical Exam Section***

Service Members initiating Command Directed Separation Examinations IAW AR 40-501 and AR 635-200 (Chapters 5-13, 5-14, 5-17, 13, 14-12a, 14-12b and 14-12c require an Escort (SGT/E-5 or above) while being in this MTF.
 
To complete Part 1 of your exam, please complete all documents provided and report to all locations you have been instructed to go to.

For Part 2, You must schedule your appointment through the Exams Office. 
Do NOT schedule any appointments through TRICARE!
                                            
For Part 2 of Your Exam, you must be 15 Minutes early, be in PT Uniform and report to the Front Desk at BLDG # 8740, First Floor, Right Wing Waiting Area for checking in.
