Physical Therapy

How to Make an Appointment

  • In order to receive Physical Therapy Services a referral first needs to be placed by your provider/ PCM.
  • Once a referral has been placed, you will be contacted within four business days to schedule your initial evaluation.
  • If you would like to reach the office sooner or are not contacted, you may:
  • Schedule in person at Building 476
  • Reach the office by phone (DSN 590-3000 or CIV 06371-9464-3000)
  • Please note referrals may take up to 24 hour to process once placed.

Come Prepared

  • Arrive 10 minutes early to all appointments.
  • Wear comfortable workout clothes or PTs (shorts if you are being seen for a knee issue).
  • If you need to cancel/ reschedule an appointment it must be prior to the appointment time.
    • Please attempt to provide 24 hours’ notice.
    • Arriving 10 minutes late or cancelling past your appointment time will result in a no-show.

Sick Call 

  • If you have experienced an acute musculoskeletal injury within the last 7 days, you may be appropriate for Physical Therapy (PT) Sick Call.
  • You must first be screened by the main clinic sick call. If the injury is appropriate, you will be provided a slip directing you to PT Sick Call. Report to the PT clinic in Building 476 (next to main clinic) with your slip after being screened.
    • Main Clinic Sick Call: 7:15 – 7:45
    • Physical Therapy Sick Call: 8:00-9:30
  • If there is no availability the same day, you may return directly to PT sick call the following day with your slip.
.Iontophoresis Patches
  • Do not need to be scheduled in advance, however you must be advised to complete round of iontophoresis patches by the physical therapist and will receive further information at that time.
  • Treatment should not occur over consecutive days, there should be at least 1 day between sessions.
Hours: 7:45 – 8:00 a.m. or 3:30 – 4:00 p.m.

Profiles

  • Discuss your need for a profile with your physical therapist at your initial evaluation.
  • Your therapist will determine the appropriate profile conditions if necessary.  
  • Any alterations to your profile must be discussed with your physical therapist.
  • If your profile will expire prior to your reevaluation, a profile extension sheet may be obtained at the front desk. It must be requested BEFORE the expiration date. Profiles will be extended at your physical therapist’s discretion.

Medical Equipment

  • Medical Equipment (insoles, crutches, etc.) will only be issued with a written note placed by your provider.

Group Classes

  • If you are selected to participate in group classes, please note they are held in the Tower Barracks Physical Fitness Center (Main Gym, Building 170) NOT the physical therapy clinic.
  • If you need to cancel or reschedule you must contact the front desk at Building 476 before the appointment time.

Dependents and Civilian Hires

  • Currently only active duty service members are treated at this facility.
  • If you have an injury, please seek care from your provider/PCM.
    • If a referral to physical therapy is warranted, your provider will refer you to physical therapy on the economy.

Closed on weekends, during lunch from 12:00 – 13:00, Federal holidays, and from 12:00 to 4:30 p.m. the first and third Thursday of the month for staff training
