Medical Readiness
Soldiers will not have their in/out processing paperwork signed off unless all MEDPROS (PHA, Immunizations, Hearing Screening, Vision Screening, or HIV Screening) are current or will not expire within 30 days of arrival/departure.
Any documentation that needs to be signed by a Provider has to be brought in person.
Eligibility: USAHC-Kaiserslautern (Kleber Clinic) Empaneled Patients
Patients may be seen for:
- Vision
- PHA/ PDHA / PDHRA / SHPE
- Hearing
- HIV
- Vision / Individual Medical Equipment (IME)
- Dental
- Immunizations
- Military Physicals: Schools, Chapter, Retirement / ETS, Commissioning (Army), ETC.
- 5118 Levy Packets
HIV
Where: Laboratory Services, USAHC-Kaiserslautern
When: Walk-in Monday-Friday, see lab hours. (You will receive a slip for proof)
Immunizations
Where: USAHC-Kaiserslautern
When: Walk-in Monday-Friday
PHA/ SHPE
Where: USAHC-Kaiserslautern
- Soldiers MUST 1st complete the online portion
- Walk-in during Soldier Readiness hours for screening and Part 1 review
- Schedule an appointment with a Provider to complete Part 2. The Medical Readiness NCO will inform you when you are ready to schedule a Provider appointment.
**Appointments will be
RESCHEDULED if online portion is not completed, and prescreening and review is not done.
PDHA / PDHRA
Once Part 1 is complete online, you may schedule an appointment to finish Part 2 with Provider.
Military Physicals
**To avoid delays or rescheduling, please bring your physical checklist, specific to the physical required, to the Medical Readiness NCO during your initial walk-in appointment. **
Where: USAHC-Kaiserslautern
To obtain a physical, you must do the following:
- Complete DD Form 2807-1 and DD Form 2808 in accordance with sample packet before coming to the clinic
- Walk-in during Soldier Readiness hours for screening and record review
- Complete requirements
- Medical Readiness NCO will verify completion
- Schedule an appointment with a Provider to complete Part 2 of the physical. The Medical Readiness NCO will inform you when you are ready to schedule a Provider appointment.
Hearing
NOTE: It can take up to two (2) weeks for MEDPROS to update.
Vision
Annual MEDPROS Screening
When: Walk-in Monday–Friday 8-11:30 a.m., 1-4 p.m.
Bring glasses if required. No contact lenses please.
Vision/ Individual Medical Equipment
- Annual Screening, Eyeglasses/Mask inserts/Combat Eye Protection
- Where: Optometry/11B Landstuhl Regional Medical Center

- Walk-ins with valid script for IME accepted, walk-ins for screening accepted (You will get a print out for proof)
- NOTE: These services are NOT available at Kleber Health Clinic; they must be done at LRMC.
AFRICOM
- Print out and fill AC Form 42
- Drop of form and orders or memo signed by Commander
- Return 72 hours later (3 working days) after 1 p.m. to receive vaccines, medications, and any other requirements.
