Medical Readiness
Soldiers will not have their in/out processing paperwork signed off unless all MEDPROS (PHA, Immunizations, Hearing Screening, Vision Screening, or HIV Screening) are current or will not expire within 30 days of arrival/departure.
Any documentation that needs to be signed by a Provider has to be brought in person.

- STEP 1 - Soldiers MUST 1st complete the online portion
- STEP 2 – Walk-in during Soldier Readiness hours for screening and Part 1 review
- STEP 3 – Schedule an appointment with a Provider to complete Part 2. The Medical Readiness NCO will inform you when you are ready to schedule a Provider appointment.
**Appointments will be RESCHEDULED if online portion is not completed, and prescreening and review is not done.
PDHA / PDHRA
Once Part 1 is complete online, you may schedule an appointment to finish Part 2 with Provider.
Military Physicals
**To avoid delays or rescheduling, please bring your physical checklist, specific to the physical required, to the Medical Readiness NCO during your initial walk-in appointment. **

To obtain a physical, you must do the following:
- Step 1: Complete DD Form 2807-1 and DD Form 2808 in accordance with sample packet before coming to the clinic
- Step 2: Walk-in during Soldier Readiness hours for screening and record review
- Step 3: Complete requirements
- Step 4: Medical Readiness NCO will verify completion
- Step 5: Schedule an appointment with a Provider to complete Part 2 of the physical. The Medical Readiness NCO will inform you when you are ready to schedule a Provider appointment.

When: By Appointment ONLY - Mondays: 8am-noon and 1pm-3pm; Fridays 1-3pm

Annual MEDPROS Screening

Bring glasses if required. No contact lenses please.

Print out and fill AC Form 42
Drop of form and orders or memo signed by Commander
Return 72 hours later (3 working days) after 1300hrs to receive vaccines, medications, and any other nrequirements.
