Medical Readiness
We will not sign your in/out processing paperwork unless all the information in the Medical Protection System (MEDPROS) is current and will not expire within 30 days of your arrival or departure. This information includes:
- Periodic Health Assessment (PHA)
- Immunizations
- Hearing screening
- Vision screening
- HIV Screening
If you need the provider to sign any documents, you need to bring them in person.
Eligibility: USAHC-Kaiserslautern (Kleber Clinic) empaneled patients
Patients may be seen for:

- Post Deployment Re-Assessment (PDHRA)
- Separation History and Physical Examination (SHPE)

- Schools, Chapter, Retirement / Expiration Term of Service (ETS), Commissioning (Army), etc.
HIV

When: Walk-in Monday-Friday (see lab hours)
You will receive a slip for proof
Immunizations

PHA/ SHPE
Where: USAHC-Kaiserslautern
- Soldiers must complete the online portion first
- Walk-in during Soldier Readiness hours for screening and Part 1 review
- Make an appointment with a provider to complete Part 2. The Medical Readiness Non-Commissioned Officer will tell you when you are ready to make a provider appointment.
We will reschedule appointments if you don’t complete the online portion and have a prescreening review.
PDHA / PDHRA
Once Part 1 is complete online, you may make an appointment to finish Part 2 with a provider.
Military Physicals
To avoid delays or rescheduling, bring the physical checklist specific to the physical you need to the Medical Readiness NCO during your first walk-in appointment.
Where: USAHC-Kaiserslautern
To get a physical, you must:
- Complete DD Form 2807-1 and DD Form 2808 (see the sample packet)
- Walk-in during Soldier Readiness hours for a screening and record review
- Complete requirements
- Medical Readiness NCO will check completion
- Make an appointment with a provider to complete Part 2 of the physical. The Medical Readiness NCO will tell you when you are ready to schedule a provider appointment.
Hearing
NOTE: It can take up to two weeks for MEDPROS to update.
Vision (Annual MEDPROS Screening)
When: Walk-in Monday–Friday 8-11:30 a.m., 1-4 p.m.
Bring glasses if required. No contact lenses please.
Vision (Individual Medical Equipment )
Annual Screening, Eyeglasses/Mask inserts/Combat Eye Protection

Walk-ins with valid script for IME accepted and walk-ins for screening accepted. (You’ll get a printout for proof)
NOTE: These services are NOT available at Kleber Health Clinic; you must go to LRMC.
AFRICOM
- Print out and fill AC Form 42
- Drop of form and orders or memo signed by Commander
- Return 72 hours later (three working days) after 1 p.m. to receive vaccines, medications, and any other requirements.
