As part of the health care team, Medical Social Workers provide psychosocial assessment and interventions to assist patients and families to achieve optimum recovery, rehabilitation and quality of life. Services include: Brief solutions focused counseling, Advance Care Planning and Long Term Care planning, grief and bereavement counseling, resource navigation and coordination of referrals to other military or civilian, community-based resources.
