Population Health
About
Population Health oversees preventive care services. We focus on wellness throughout a patient’s life. We also measure the effectiveness and quality of healthcare given in the clinic. Population Health manages the Healthcare Effectiveness Data and Information Set (HEDIS) report. HEDIS includes defined health measurements to keep our military members and their families well. Leading health care systems use the same health measures which fit the national definition of access, safety, and quality care. Key HEDIS measures include:
- Breast cancer screening (beginning at age 40)
- Cervical cancer screening (beginning at age 21)
- Chlamydia screening
- Colon cancer screening (beginning at age 50)
- Diabetes care
- Well-child visits
- Low back pain imaging
Health promotion/disease prevention services
- We monitor patients to make sure they get health care screenings and have top health care. Patients can expect phone calls, patient reminder letters, or messages through the MHS GENSIS Patient Portal.
- Individual patient education and pre-procedure counseling visits are available from the primary care provider or team.
Our goal is to foster a collaborative approach between patients and their health care teams to prevent disease and increase wellness.
