*An authorization is required and can be requested prior to or the next business day after your visit*
What types of visits will be covered?
Upper respiratory infections, ear infections, nausea, vomiting, diarrhea, first trimester concerns in pregnancy, UTIs, acute behavioral health concerns, minor cuts and injuries
What types of visits will not be covered?
Yearly exams, sports and school physicals, medication refills, symptoms lasting more than two weeks, broken bones, OB patients over 12 weeks gestation, surgical issues such as appendicitis, active duty profiles or convalescent leave
