Patient Centered Medical Home
The Vilseck Health Clinic is proud to serve as a Patient Centered Medical Home (PCMH). In a Medical Home, you partner with a personal primary care provider, along with his/her team of nurses (RN and LPN) and Medics. This core team works with you over time to take care of new health concerns as they arise, ensure delivery of preventive screening and services, manage chronic health problems, and promote a spirit of health and wellness. The Vilseck Health Clinic has two major teams with six core teams.
As part of our commitment to improved access, we offer a broader range of options to follow-up on health issues and concerns, share and update health information, and coordinate care delivery. Along with traditional face-to-face office visits, we offer enhanced telephone communication and web-based communication such as secure email through TOL Secure Messaging.
We will go the extra mile to ensure you get care from your own healthcare team, the team that knows you and is responsible for your care. The relationship between you and your healthcare team is the heart of the Medical Home for your care.
Your Responsibilities
In the Medical Home, you are part of the team. As part of the team, you share responsibility for your health. We need you to take an active role in your care by providing us with a complete medical history, and medical records from care received at outside facilities, asking any question you have, and participating in the decisions that affect the health of you and your family.
