Patient Centered Medical Home

The Grafenwoehr Health Clinic operates under the Patient Centered Medical Home model. This model serves to facilitate Patient Centered care that provides continuity in the health care team that delivers care to our Beneficiaries. The clinic has two major teams (Blue and Red) with three core teams assigned to each. Each team consists of a dedicated Provider, Nurse and Medic. You have the option of choosing your Primary Care Team. Please visit our clinic's TRICARE Enrollment Office to request your Primary Care Manager.
