Medical Physical Exams/Readiness
We will not sign your in/out processing paperwork unless all the information in the Medical Protection System (MEDPROS) is current and will not expire within 30 days of your arrival or departure. This information includes:
- Periodic Health Assessment (PHA)
- Immunizations
- Hearing screening
- Vision screening
- HIV Screening
If you need the provider to sign any documents, you need to bring them in person. pPatients may be seen for:
- PHA
- Pre-Deployment Health Assessment
- Post Deployment Re-Assessment
- Separation History and Physical Examination
- Hearing Conservation
- HIV
- Vision
- Dental
- Immunizations
- 5118 Levy Packets
- Physicals: Military, School, Separation, Retirement / Expiration Term of Service, Commissioning, etc.
HIV
- Where: USAHC-Kaiserslautern Laboratory Services
- When: Walk-in Monday-Friday (see lab hours)
- You will receive a slip for proof.
Immunizations
- Where: USAHC-Kaiserslautern
- When: Walk-in, Monday - Friday
PHA
Where: USAHC-Kaiserslautern
- Soldiers must complete the online portion first on the Medical Protection System.
- Make an appointment with a provider to complete Part 2. Call the appointment line or Kleber Clinic to schedule your appointment.
Other PHA completion option - Call the Virtual Medical Center Europe / Virtually Integrated Patient Readiness and Remote Care Clinic (VIPRR Clinic) to schedule:

- Please ensure you have completed your portion of the PHA BEFORE scheduling a virtual PHA appointment with VIPRR.
Separation History and Physical Exam - SHPE
Where: USAHC-Kaiserslautern
- Soldiers must complete the online portion first on the Medical Protection System.
- Walk-in during Readiness hours for screening and review.
- Readiness NCO will tell you when you're ready to make a provider appointment after pre-requisites are complete.
We'll reschedule appointments if you don't complete the online portion before your screening review.
PDHA / PDHRA
Once Part 1 is complete online on MEDPROS, you may make an appointment to finish Part 2 with a provider.
Military Physicals
To avoid delays or rescheduling, bring the physical checklist from the requesting representative to the Medical Readiness NCO during your first walk-in appointment.
Where: USAHC-Kaiserslautern
To get a physical, you must:
- Complete DD Form 2807-1 and DD Form 2808
- Walk-in during Soldier Readiness hours for a screening.
- Readiness NCO will check completion
- Make an appointment with a provider to complete Part 2 of the physical. The Medical Readiness NCO will tell you when you are ready to schedule a provider appointment.
Hearing
NOTE: It can take up to two weeks for MEDPROS to update.
Vision (Annual MEDPROS Screening)
When: Walk-in Monday - Friday 8-11:30 a.m., 1-4 p.m.
Bring glasses if required. No contact lenses please.
Vision (Individual Medical Equipment )
Annual Screening, Eyeglasses/Mask inserts/Combat Eye Protection
Where: Optometry/11B Landstuhl Regional Medical Center

Walk-ins with valid script for IME accepted and walk-ins for screening accepted. (You’ll get a printout for proof)
NOTE: These services are NOT available at Kleber Health Clinic; you must go to LRMC.
AFRICOM Official Travel
- Print out and fill AC Form 42
- Walk-in to clinic during Readiness hours for screening to receive vaccines, prescriptions and any other requirements.
- Don't forget to bring your orders, a memo, or travel itinerary signed by your Commander.
- Note: Certain prescriptions/vaccines require no less than 10 days lead time.
